The way I see it, it's all about risk.

Hourly rate. The customer takes all the risk.

Fixed rate. The consultant takes all the risk.

I have not found much middle ground.
There are different strategies to try and reduce risk, but who carries the 
risk, on my experience, doesn't change easily.
